Well I scored enough on the social club laundry mat from rock star online to unlock the first costume...I syned my DS but can't find the costume. I heard its a multiplayer costume? What does this mean? How is this costume I won integrated in the game?

I assume it's something you'll see when playing multiplayer, and multiplayer only. I haven't used CTW's multiplayer, so I don't know anything about it.
It probably is no less than a color change of your character's clothes.

Is the multiplayer on this game only one to one with a DS? Or can you play with anyone around the world?
In other words is it ad hoc or infanstructure?
